PT-141 is the research and generic name for bremelanotide, a synthetic heptapeptide. The FDA approved it in 2019 as Vyleesi for premenopausal women with hypoactive sexual desire disorder (HSDD). Its mechanism differs from phosphodiesterase inhibitors such as Viagra or Cialis.
PT-141 is related structurally to melanotan-2, another melanocortin agonist, but with different selectivity and clinical history. Melanotan-2 was studied for tanning and erectile-function models. PT-141 was developed toward the arousal and desire pathway that led to the HSDD indication.
Research context: PT-141 development involved Palatin Technologies, and the compound sat in clinical development for over a decade before the FDA approval of Vyleesi in 2019. The DESIRE (female sexual desire) clinical trial programme drove regulatory approval.
Mechanism: melanocortin receptor signalling
PT-141 works as a selective agonist of the melanocortin-4 receptor (MC4R) . MC4R is a G-protein-coupled receptor in hypothalamic and related brain regions tied to sexual motivation and arousal. This is a central nervous system mechanism. It does not primarily alter genital blood flow, which is why it differs from phosphodiesterase inhibitors.
The melanocortin pathway is multifunctional. It is studied in appetite, energy homeostasis, immune signalling and sexual behaviour. MC4R activation in hypothalamic nuclei increases sexual motivation in animal models. Human trials reported changes in subjective arousal and desire under study conditions.
Greater MC4R selectivity reduces some off-target melanocortin effects compared with broader agonists. Skin darkening is still a documented adverse event in a subset of trial participants.
Clinical evidence: the DESIRE trials
PT-141’s path to approval was the DESIRE trial programme : Phase 3 randomised controlled trials in premenopausal women with HSDD. The primary endpoint was an increase in satisfying sexual events per month, a patient-reported outcome rather than a mechanistic surrogate.
Trial design snapshot: Participants were randomised to placebo or PT-141 across studied dose arms. Trials reported statistically significant improvements versus placebo on desire, arousal and satisfying sexual events. Effect sizes were modest but directionally consistent across the programme.
Nausea was common in the trials and was dose-related. Blood pressure elevations were also observed and required monitoring in the clinical programme. The trials enrolled hundreds of participants over several months and formed the evidence base for the HSDD indication in women.

Contraindications and cautions include uncontrolled hypertension, coronary artery disease, and concurrent use of strong CYP3A4 inhibitors.
